Our Journey
Built to Transform Specialty Cardiac Care
ISHI was founded by Dr. Ajay Srivastava, an advanced heart failure and transplant cardiologist, after seeing firsthand how difficult it is for clinical teams to continuously manage complex cardiac patients between visits. ISHI was built to extend specialist-led care beyond the clinic by combining multidisciplinary clinical care, structured pathways, connected data, and AI.

Founded Around a Clinical Problem
ISHI Health was founded with a mission to make continuous, personalized specialty cardiac care accessible beyond traditional clinic visits.
- 2024
Building the Clinical and AI Foundation
ISHI developed its virtual heart failure care model and began developing machine-learning capabilities in collaboration with Mayo Clinic Platform, while deploying its platform with cardiology practices and evaluating early clinical outcomes.

Scaling Continuous Cardiac Care
ISHI expanded its clinical partnerships and multidisciplinary care programs, supporting a growing population of heart failure patients through remote monitoring, medication support, structured care pathways, and proactive care between visits.

Expanding the Specialty Care Platform
ISHI is expanding CardioCommand and its AI-enabled care infrastructure to support heart failure and cardiometabolic populations across clinical practices, health systems, and value-based care models, including post-discharge care, cardiac device monitoring, and expanded access to specialty care.
